You are being invited to participate in an online study which explores the experiences of Canadian Armed Forces (CAF) Veteran amputees and Family members of CAF Veteran amputees access and use of resources and services. The purpose of the current study is to engage CAF Veteran amputees and Family members of CAF Veteran amputees multidimensional service use and needs in order to identify service gaps and plan for the development and implementation of resources, services, and supports.

Survey
This study will use a mixed-methods, cross-sectional, dyadic investigation, to conduct a needs assessment of CAF Veteran amputees and Family members of CAF Veteran amputees. You will be asked to participate in a 30 - 45 minutes online survey on Lawson Research Electronic Data Capture (REDCap).
